It always bothers me that these trains are allowed to operate with doors open and people standing right there liable to fall out. Is there a lack of air conditioning? A real safety issue. How often are people hurt/killed this way?

Your concern for the open doors and people leaning out is very much valid point but Indian Railways has to cope with enormous number of passengers at affordable rates. It is not that they are not bothered for the safety. They regularly announce about not to travel on foot-boards and also try to see the doors are clear at train starting stations. But in subsequent stations they are just helpless due to enormous amount of crowd. Subsequently it becomes habitual for some persons to travel in the open doors of moving trains. Most of the trains here have right from un-reserved class coaches to higher end aircon class coaches. Normally this is observed in 1 or 2 end coaches which are general, non-airconditioned, unreserved coaches having lowest ticket fare. In other reserved coaches the Ticket examiner and police try to keep the doors closed while journey but again in crowded situations they are helpless.
