I wonder if this was a remote control yard job? If so, should this technology be upgraded to include a locomotive camera view, so the ground based conductor / engineer can see any obstacles in front of the move?

Derails Work! If this was a 'conventional' crew, the engineer is a fault for 'overlooking' the derail. If it was a 'remote' job the Remote Operator is responsible for knowing the limits he must operate his locomotives within. Somebody(s) will be getting a undesired vacation.
